CTDHS Enrolment Form for Kindergarten enrolments and students who have never been enrolled in a Tasmanian Government School.
Form A to be completed by Parent/Guardian for non-prescribed medication.
Form B to be completed by Doctor/Pharmacist/Practice Nurse for presribed medication.
STAS - Student Assistance Scheme
Online Application for STAS. Families with a Health Care Card are eligible to apply
