Parent Teacher Online
CTDHS Enrolment Form for Kindergarten enrolments and students who have never been enrolled in a Tasmanian Government School.
Change of student personal details such as residential and/or mailing address, emergency contacts and new medical conditions.
Form A to be completed by Parent/Guardian for non-prescribed medication. Both Form A (Parent/Guardian) and Form B (Doctor/Pharmacist/Practice Nurse) to be completed for prescribed medication.
The Student Assistance Scheme (STAS) provides assistance for low income families towards the cost of levies. Families that have been approved STAS for 2018 will be contacted by DoE in writing advising them of their approval status for 2019
